  • blood pressure screening and assessment
  • referral to doctors
  • disease case management and treatment

The Georgia Hypertension Management and Outreach (HMO) Program also provides counseling on lifestyle modification topics including:

  • healthy eating
  • physical activity
  • tobacco cessation
  • weight management
  • medication adherence
