About the DPH Home Visiting Program

We support moms and babies during pregnancy and after birth – from pregnancy through the baby’s first year – a time when guidance, care, and connection can make a real difference.

The program is also designed to help families early, before minor issues become serious challenges or complications. Families do not need to be in crisis to qualify. Any high-risk pregnant woman or infant is eligible. Common risks include:

  • Health concerns like preeclampsia or high blood pressure
  • Feeling stressed or overwhelmed
  • Limited support system
  • Difficulty keeping provider appointments

What We Do

One of our public health nurses or trained community health workers visits a participating family wherever they feel most comfortable. During these visits, we focus on what matters most for each family’s health, from identifying warning signs early, answering health questions, and connecting patients with local support and resources.  

Our nurses and community health workers partner closely with patients’ healthcare providers to check in with patients between appointments.

Program Information and Enrollment

The program is of no cost to patients or providers. Once a family is referred, the DPH Home Visiting team manages outreach, enrollment, and follow-up. You can make a referral through the DPH Home Visiting Program Referral Form below or by contacting our office.
