Soil Scientist Certification
New Certified Soil Classifier (Soil Scientist)
NOTE: Geologists and Engineers have a different application/set of requirements (see below tab).

(includes $100 late fee; if you only pay $400, your late renewal will be denied)

Body Artist Certification
Get Certified for Tattooing, Piercing or Microblading
Please call your local County Environmental Health Office to schedule an appointment to take the exam(s) for Body Artist Certification.
The exams are based on the Rules and Regulations for Body Art (PDF, 636.22 KB) .
Upon passing the exam, the Body Artist Certification Application will be emailed to your personal email account.Example Application (PDF, 168.3 KB) (not for use)

Renew Certification
NOTE: Renewals for the next year have started. You should have received an email from the DPH Body Art Program ([email protected]) that has instructions and a link to the renewal application on DocuSign. If you did not receive the email, please check your spam/junk folder and if you can't find it, reach out to the Body Art Program.
Applications are reviewed within 20 business days.
IMPORTANT NOTES:
- All required training certificates, including Bloodborne Pathogens, First Aid, and CPR training, must not expire within six months of your certification renewal.
- If an applicant previously declined the Hepatitis B vaccination and wishes to maintain that choice, they must sign a new Hepatitis B Declination Letter.

Tanning Facility Registration
After January 1, 2011, no person shall establish, maintain, or operate a tanning facility in the state without first having registered with the Department of Public Health.

Registration is required annually, starting Jan 1st of every year.
Fee: $25 for each facility + $15 for each tanning device.
Once an initial application and payment are received, annual renewals can be done making the registration payment online.
