WIC Eligibility Assessment

- I currently reside in the State of Georgia
- I am applying for someone who is currently pregnant
- I have, or someone in my household has delivered a baby within the last 6 months
- I am, or someone in my household is breastfeeding an infant under 12 months old
- I am applying for a child/children less than 5 years old
- I am currently receiving Medicaid, Food Stamps/SNAP or TANF

WIC Application
Complete a WIC application at the Georgia Gateway website. If you are eligible for benefits, your clinic will contact you with an appointment for your Nutrition Assessment and food benefits.
