The Georgia Department of Public Health's Newborn Screening Program provides copies of newborn screening records, including sickle cell screening results, at no cost.

You may request your newborn screening record directly through the Newborn Screening Program. You may also be able to obtain a copy from your healthcare provider or your local public health department.

Parents or legal guardians may request the newborn screening records of children who are 17 years of age or younger.

  1. Gather What You'll Need

    Before submitting your request, have the following information and documents ready:

    • Information about the individual whose record is being requested:
      • Full name
      • Date of birth
      • Birth hospital
      • Mother's full name at the time of delivery (maiden name, if applicable)
    • Valid government-issued identification for the person making the request. Acceptable forms of identification include:
      • State-issued driver's license with photo and current address
      • State-issued identification card with photo and current address
      • U.S. passport or U.S. passport card
    • Your contact information, including:
      • Mailing address
      • Email address
      • Telephone number
    • Completed Authorization for Release of Newborn Screening Report form  (PDF, 1.1 MB) (PDF, 1.1 MB). Download and complete the form before submitting your request.

  2. Request a Newborn Screening Record

    Follow these steps to submit your request:

    1. Complete and sign the Authorization for Release of Newborn Screening Report  (PDF, 1.1 MB) form.
    2. Email the following to [email protected]:
      • The completed and signed authorization form
      • A copy of your valid government-issued identification
    3. Enter "NBS Record Request" in the subject line of your email.

    Important: Requests cannot be processed without a valid government-issued photo ID from the person requesting the newborn screening record.

  3. What Happens Next?

    After you submit your request:

    • Please allow at least 3 business days for your request to be processed. Requests are processed during normal business days only and are not processed on weekends.
    • You will receive your newborn screening record by encrypted email from [email protected]. If you do not see the email in your inbox, check your Junk or Spam folder.
    • Open the encrypted email by entering the encryption code when prompted.
