Healthy Communities Initiative
The Healthy Communities Initiative is a statewide effort to create environments that support healthy living and reduce the burden of chronic disease. Funded by the Chronic Disease Prevention Section (CDPS), this initiative strengthens Georgia’s public health infrastructure by supporting local public health districts in their efforts to improve community well-being.
CDPS provides funding, leadership, technical assistance, and training to help local health districts implement effective policy, systems, and environmental (PSE) changes. These changes are designed to promote healthier lifestyles and create lasting improvements in the places where Georgians live, learn, work, and play.
Through this collaborative approach, the Healthy Communities Initiative works to:
- Prevent and reduce chronic diseases
- Increase access to healthy foods and physical activity
- Promote tobacco-free living
- Strengthen partnerships that advance health equity
Together, we are building healthier, more resilient communities across Georgia.
Local health departments develop and leverage Community Health Assessments (CHAs) and Community Health Improvement Plans (CHIPs) to identify key health issues. They work in partnership with health care providers, community organizations, and coalitions to strengthen local efforts and maximize public health impact across Georgia.
