Low THC Oil Information for Physicians

Below you will find the key information that physicians need to know to certify patients and caregivers for a Low THC Oil card and update the Registry. Please refer to the FAQs below or contact the Georgia Low THC Oil Program with any further questions.

Certifying patients for a Low THC Oil card:

  1. Register and log into your Low THC Oil Registry account
  2. Direct patients and caregivers to fill out the Download this pdf file. Low THC Oil Waiver and have it notarized if they meet the eligibility requirements
  3. Complete the Download this pdf file. Low THC Oil Certification
  4. Enter patient and caregiver information, and upload photos of the front and back of their valid driver’s license(s), state-issued identification card(s), passport(s), military ID(s), or other valid forms of identification into the Registry
  5. File the Low THC Oil waiver and certification in the patient’s medical record

Updating the Registry:

  • Update patient and caregiver information for Registry accuracy as needed
  • Cancel patient’s cards in the Registry if they no longer use Low THC Oil, have moved out of state, or have died

Document physicians need to fill out and submit to certify patients to get a Low THC Oil card

By signing the certification, physicians consent to be in good standing to practice medicine in Georgia, have a bona fide physician-patient relationship with the patient in compliance with state statutes, have assessed this patient’s medical history and current medical condition, have performed or reviewed appropriate diagnostic tests in making the above-indicated diagnosis, and authorize the patient to be eligible for the use of low THC oil as provided in Georgia law. This authorization is not a prescription.

  • What is your role in the Low THC Oil patient certification process?

    As a physician fully licensed to practice in Georgia, your role in the Low THC Oil process is:

    1. Registering in the Low THC Oil Registry
    2. Explaining the Low THC Oil certification processes to patients and/or caregivers
      1. Patients/caregivers must complete the Low THC Oil Waiver and have it notarized prior to certification
    3. Filling out the certification form and adding patients and their caregivers to the Low THC Oil Registry
    4. Canceling patient’s cards in the Registry if they are no longer using Low THC Oil, have moved out of state, or have died
    5. Completing required Registry reports as directed by Georgia law (Official Code of Georgia Annotated, OCGA §31-2A-18)
  • How can you certify a patient for a Low THC Oil card?
    1. Register and log into your Low THC Oil Registry account

  • What do you need to know before you certify a patient?
    • Patients must be Georgia residents
    • Patients must have one or more qualifying conditions
    • You must have a standing doctor-patient relationship with patients you certify
    • You are not required to certify patients; the certification decision is based entirely on your judgment
    • You must upload both the front and back of the patient’s and caregiver’s valid driver’s license(s), state-issued  identification card(s), passport(s), military ID(s), or other valid forms of identification into the Registry
    • You must file the Low THC Oil Waiver and Certification in the patient’s medical record
  • Are you required to certify a qualified patient?

    No. It is entirely your decision whether to certify a patient.

  • Is certifying a patient the equivalent of writing them a prescription for Low THC Oil?

    No. The act of certifying a patient is simply an acknowledgment that you have an established doctor-patient relationship, have examined them, and determined they have one or more of the medical conditions set forth in the law. The Low THC Oil Certification approved by the Georgia Composite Medical Board specifically states that it is not a prescription.

  • Will you be prosecuted by Georgia or federal law enforcement for registering eligible patients?

    No. The registration process has been established by Georgia law and it does not violate any state or federal laws.

  • Will you lose your medical license for registering eligible patients?

    No. The Georgia Code 16-12-231 provides protection from arrest, prosecution, or disciplinary action from professional licensing boards for physicians who certify patients to the Low THC Oil Registry. 

  • Why is it important for you to accurately utilize the Registry?

    It is critical to accurately utilize the Registry to:

    • Support legal, uninterrupted card use for qualified patients

    • Help safeguard Low THC Oil use for only those who legally qualify

    • Provide up-to-date Low THC Oil data reporting
