Low THC Oil Information for Patients and Caregivers

Requesting a Low THC Oil card:

Consult with your physician about obtaining a Low THC Oil card if you have a qualifying condition(s)
If approved by your physician, fill out the Low THC Oil Waiver and have it notarized. Provide the waiver and a valid driver’s license, state-issued identification, passport, military ID, or other valid forms of identification for you and your caregiver to your physician
Pay for your Low THC Oil card via our secured payment portal or money order. Once your payment is complete, your card will be shipped to the mailing address on file. If you do not have a permanent or secure home address or use a P.O. Box, your card will be shipped to the health department location nearest your residence. Please allow up to 10 business days to receive your card

You will need to fill out and sign an authorization form if someone other than yourself or your caregiver is picking up the card on your behalf
You will need to show a valid driver’s license, state-issued identification card, passport, military ID, or other valid form of identification to pick up your card

Each Low THC Oil card is $30. You will receive an email with a link to the payment portal. There is a $3.75 service fee when using the payment portal. If you are unable to pay via our payment portal or do not have access to an email address, you can mail a money order in the amount of $30 to:

Low THC Oil Waiver
Waiver patients and caregivers need to fill out, sign, and have notarized to get a Low THC Oil card
The use of cannabinoids and THC containing products have not been approved by the FDA and the clinical benefits are unknown and may cause harm. By signing the waiver, patients and caregivers are voluntarily agreeing and consenting to treatment through the use of cannabinoids and THC containing products and waive any rights to actions against the physician and the State of Georgia for the use of cannabinoids and THC containing products.

Form cardholders need to complete and sign authorizing someone to pick up their card from the health department on their behalf.

How do you apply for a Low THC Oil card?
1. Consult with a registered physician about getting a Low THC Oil card if you have a qualifying condition(s)

How long does it take for the Low THC Oil card to be ready? Where do you pick it up?

Low THC Oil cards can take up to 10 business days from the time your payment is processed. To check your card status, click here.
Cards will be printed and shipped to the cardholder’s mailing address on file. If the cardholder does not have a permanent or secure home address or uses a P.O. Box, the card will be sent to the health department location nearest the cardholder’s (patient’s or caregiver’s) residence. The Department will contact cardholders when their card is ready. To pick up the card, cardholders must provide a valid driver’s license, state-issued identification, passport, military ID, or other valid forms of identification.
What if more than one person is caring for the child or adult?

If there is more than one caregiver, each caregiver may apply for a separate card. Each card costs $30.
Can a cardholder authorize an individual to pick up your Low THC Oil card?

For cards shipped to a health department location, patients and caregivers may authorize an individual to pick up their Low THC Oil card from the health department on their behalf with a signed authorization form . The individual picking up the card should have identification for themselves as well as the cardholder.
Which health departments distribute Low THC Oil cards to certified patients and/or their caregivers?
For patients and caregivers without a permanent or secure home address or with a P.O. Box, their cards will be shipped to one of the 43 health department locations nearest their residence.

How much does the Low THC Oil card cost, and is it covered by health insurance? When do you pay for it?

The fee for a Low THC Oil card is $30 per card, and it is not covered by health insurance. Cardholders will receive an email with a link to our secure payment portal. There is a $3.75 service fee when using the payment portal. If a cardholder is unable to pay via the payment portal or does not have access to an email address, they can mail a money order in the amount of $30 to:

How long is the Low THC Oil card valid?

The Low THC Oil card is valid for five years from the date your payment is processed. The expiration date is printed on the front of the card. After that time, you must consult your physician to renew your card.
What happens if you lose your card, or the information on your card is wrong or outdated? Can it be corrected?

Where can you buy Low THC Oil after receiving your card?

You can go to the Georgia Medical Cannabis Commission’s website to find locations that sell Low THC Oil.
Your employer tests for drug use, including cannabis. Can they test you if you are a medical cannabis patient? Can they fire you if you use medical cannabis?

Low THC Oil can show on drug tests and is used at the patient’s discretion. Georgia law does not prevent an employer from testing for the use of cannabis (for any reason) or taking action against an employee who tests positive for the use of cannabis (for any reason).
